Description
Creates a Slate Form based on the id of the form.
Used to place a slate form on a standard page, or a landing page in a Text Box. Standard pages should use the stacked layout using the stacked parameter.
Syntax
[slate id="{ID}" stacked]
Parameters

|---|---|---|
| id | Yes | Slate Form ID |
| stacked | No | Sets up a stacked (vertical) vs a horizontal layout. |
